The complex medical and sub-acute care program at Johns Hopkins Bayview assists patients with medically complex conditions, particularly wound care, who cannot return to the community safely after discharge from the acute hospital setting. Services are designed to assist with increasing functional status, while training caregivers to prepare for a smooth discharge. Patients are treated on the Chesapeake Unit in the Johns Hopkins Bayview Care Center using an interdisciplinary team approach to patient care.
