Elderly patients who need long-term care often prefer to receive much of their care in the comfort of their own homes and communities. Hopkins ElderPlus is a community-based program designed to help frail, elderly patients receive needed care while remaining in familiar surroundings.

Opportunities are available in an interdisciplinary team of health professionals in various settings, including adult day care, home care, and a primary care clinic.
