Case management staff are active throughout Johns Hopkins Bayview, from the neonatal ICU to general medicine. They provide a wide range of vital services, including discharge planning, care coordination, disease management and utilization management.

Using a high level of autonomy, our case management nurses excel at helping patients find their way through the health care system, finding services for patients who are having difficulty obtaining treatment. They find creative solutions to patient and system issues that pose barriers to successful patient outcomes.
